Experienced Sonoma County physician leader brings expertise in primary care, pediatrics, clinical innovation, and health informatics to Alliance Medical Center.
For more than 50 years, Alliance Medical Center has provided accessible, high-quality healthcare to individuals and families throughout northern Sonoma County. Today, the nonprofit Federally Qualified Health Center serves approximately 21,000 patients and provides comprehensive medical, dental, behavioral health, optometry, wellness, and supportive services designed to meet the needs of the diverse communities it serves.
As Alliance looks toward the future of community-based primary care, the organization is pleased to welcome Scott Holmes, DO, as Chief Medical Officer, bringing an accomplished Sonoma County physician leader with a passion for compassionate, patient-centered care and a strong background in clinical innovation.

Dr. Holmes is board certified in both Internal Medicine and Pediatrics and brings extensive experience in clinical leadership, primary care, pediatrics, and health informatics. Most recently, he served as Medical Director of Primary Care at Petaluma Health Center, where he provided clinical and operational leadership across a multi-site Federally Qualified Health Center. He previously served as Director of Clinical Informatics and Associate Medical Director and continues to practice as a primary care physician. Dr. Holmes has also served as a pediatric hospitalist and Chair of the Pediatrics Department at Providence Petaluma Valley Hospital.
“We are thrilled to welcome Dr. Holmes to Alliance,” said Sue Labbe, Chief Executive Officer of Alliance Medical Center. “Scott brings a wonderful combination of clinical expertise, thoughtful leadership, innovation, and a genuine commitment to patients and the people who care for them. His training in both internal medicine and pediatrics aligns beautifully with our commitment to caring for individuals and families throughout every stage of life. Just as importantly, he understands the unique role community health centers play in creating access, building trust, and improving the health of the communities we serve.”

Dr. Holmes said Alliance’s history, community roots, and willingness to innovate were central to his decision to join the organization.
“I love Alliance’s history and what the organization stands for. It is deeply rooted in the community while remaining nimble and innovative in how it serves the patients of northern Sonoma County. The opportunity to help strengthen and grow that mission as Chief Medical Officer was incredibly appealing to me.”

In his new role, Dr. Holmes will provide clinical leadership across Alliance while continuing to care directly for patients. A central focus of his leadership will be strengthening the systems that support both excellent patient care and a sustainable primary care workforce.
“I want to find ways to support clinicians so they can practice medicine the way they envisioned when they chose this profession: delivering compassionate, patient-centered care without being overwhelmed by administrative burdens. By investing in our clinical teams and support staff, we can provide the highest-quality patient care while fostering the work-life balance necessary to sustain a strong and resilient primary care workforce.”

Dr. Holmes brings particular expertise in using health informatics and technology to improve clinical care and reduce administrative burden. He is an Epic Physician Builder and has led initiatives focused on electronic health record optimization, population health, digital health equity, virtual care, and the thoughtful integration of new technologies into clinical practice. His previous leadership experience includes implementing technology designed to give clinicians more time to focus on their patients and strengthening clinical workflows across multiple service lines.
At the heart of his work, however, is a commitment to relationships between patients, families, and their healthcare teams.
“What brings me the most joy in practicing medicine is building lasting relationships with patients and families. I look forward to caring for children, parents, and grandparents, providing longitudinal care that allows trust and meaningful relationships to develop over time.”
— Scott Holmes, DO
His combined training in Internal Medicine and Pediatrics uniquely prepares him to provide care across the lifespan, from infancy through older adulthood.
“Some of my fondest memories are caring for multi-generational families over time. Whether it is watching pediatric patients grow into young adults, guiding them through adolescence, or caring for a mother and infant simultaneously, I enjoy the opportunity to support individuals and families through different stages of life.”
— Scott Holmes, DO
Dr. Holmes completed his combined Internal Medicine and Pediatrics residency through ChristianaCare, Thomas Jefferson University, and Nemours after earning his Doctor of Osteopathic Medicine from Philadelphia College of Osteopathic Medicine. He also holds a Master of Science in Cancer Biology from the University of California, San Diego, and a Master of Arts in Teaching and Education from the University of Southern California.
Dr. Holmes and his family have called Sonoma County home for the past seven years. Outside of medicine, he is an avid sports fan and enjoys exploring the North Bay with his family and cheering on his two children at their sporting events. His family is also involved with Guide Dogs for the Blind through its K9 Buddy Program, which connects children who are visually impaired with specially selected dogs.
“We are excited not only about the experience Scott brings to Alliance today, but also about the future we can build together. His commitment to patients, clinicians, innovation, and community reflects who we are as an organization and where we want to go. We are incredibly fortunate to have him on our team.”
